Because this information is used for your appointments and provision of healthcare, you must provide us with accurate and complete information and notify us if there are changes.

Our application and website also use cookies and similar tools to access and store information. This allows us to recognize your browser to provide you with information.

Review and Update Information: You can review and update your information by logging in and altering your account information. You can also contact us to update any information. You may terminate your account, but we cannot delete all of your information. As a healthcare provider, we are required by law to keep certain information.

Unsubscribe: You can unsubscribe from our marketing emails at any time. This can be done by contacting us or by selecting “unsubscribe” in the email. We may still send you emails that are related to your account.

California’s “Shine the Light” law: California residents may request a detailed disclosure of how personal information was shared for direct marketing reasons. This disclosure will contain the third parties we have shared information with within the past 12 months. California residents may request this disclosure once a year.

Updates: We may make updates to this policy. Please note the revision date at the bottom of this document. The revisions will be effective the same date they are posted. If material changes are made, we will notify you, such as through a message in the application or via email. You should review this policy often, as changes may be made periodically.
